Approximately 2 weeks before
Before your assessment day
Your Health Navigator reaches out to coordinate everything. You complete your bloodwork and any required imaging in advance — so your physician has reviewed all results before you arrive.
Fasting Bloodwork
A comprehensive panel drawn 2 weeks before your assessment day. Fasting for 8–12 hours is required for this appointment only — not for the assessment day itself.
Health Navigator Intake
Your Health Navigator contacts you to confirm your appointment, answer questions, send your preparation checklist, and coordinate any additional pre-assessment requirements.
What to Bring
Your provincial health card (OHIP), a government-issued photo ID, a list of current medications and dosages, and any previous lab results or imaging you have access to.
Approximately 4 hours
Your assessment day
A dedicated registered nurse is assigned to you for the full day. You move through five clinical appointments — each with dedicated, unrushed time — then close with your physician debrief.
Arrival & Guided Assessment
Your assigned RN meets you, reviews your health history, takes vitals, and guides you through the full assessment day — including vision and hearing screening, ECG, and coordination of all appointments.
Clinical Examination & Risk Review
A comprehensive physical examination with your physician, including a review of your bloodwork results, cardiovascular risk, hormonal status, and any concerns specific to your health profile.
Nutrition & Metabolic Health
A dedicated nutrition assessment covering your dietary patterns, metabolic health, energy balance, and lifestyle factors — with personalized recommendations included in your Passport for Health.
Physical Capacity & Musculoskeletal Assessment
An evaluation of strength, flexibility, posture, balance, and physical capacity — identifying musculoskeletal risks and physical performance benchmarks with a targeted plan for improvement.
Mental Health & Resilience
A private, structured session evaluating stress load, sleep quality, mood, cognitive function, and emotional resilience — with practical strategies tailored to your life and work demands.
Your Results & Passport for Health
Your physician reviews all findings with you directly — bloodwork, clinical examination, and allied health recommendations — and presents your personalized written health plan.

Your Passport for Health
At the close of your assessment day, you leave with a personalized written health plan — not a stack of lab results you have to interpret yourself, but a clear, structured document your physician builds with you during your debrief.

After your assessment
Ongoing support & follow-up
Your Health Navigator coordinates any follow-up referrals, investigations, or specialist appointments identified during your assessment — so nothing falls through the cracks.
Referral Coordination
If your assessment identifies areas requiring specialist follow-up, your Health Navigator coordinates referrals and tracks completion on your behalf.
Family Physician Summary
With your explicit consent, a clinical summary can be shared with your family physician to support continuity of care. Your health information is never shared without your written consent.
Annual Re-Assessment
Health changes over time. Many clients return annually to benchmark their progress, reassess risk, and update their Passport for Health as their goals evolve.
